Your First Visit

At Artisan Smiles, we treat patients of all ages in a warm and comforting environment. Your first visit at Artisan Smiles will consist of a consultation with our experts. We will examine and diagnose your condition during this comprehensive evaluation. We will then design a specific treatment plan tailored to your individual needs and goals. Your safety, comfort, and satisfaction are our utmost priority. 

Please arrive a few minutes prior to your scheduled appointment to allow time to fill out the appropriate forms. To save time, feel free to fill out the forms online. 

To make the most of your initial visit and consultation, please bring the following items with you: 

• Valid Photo ID or Parent ID for minors
• Valid insurance card
• Referral sheet
• Radiographs (xrays) within the last 6 months if available 
• New Patient Paperwork (Fill out online or see below)

All patients under 18 must be accompanied by a parent or legal guardian to all appointments.

New Patient Forms

We know your time is valuable. In order to make the check-in process more convenient, you can download our new patient forms, print them out, and bring the completed forms to our office to expedite the paperwork at your first visit.
